バベシア症の調査 ― ウィスコンシン州、2001~2015年
バベシア症は主に原虫のBabesia microtiが赤血球に寄生して生じる人獣共通感染症である。ライム病やアナプラズマ病の病原体のように、Babesia microtiはアメリカ北東部および中西部北部に特有で、通常クロアシマダニのIxodes scapularisにより媒介される。バベシア症は通常、軽度から中等度の疾患であるが、高齢者および免疫不全の人では重症のマラリア様疾患を形成し、迅速に治療しないと致命的である。バベシア症の最も一般的な初期徴候および症状(発熱、倦怠感、悪寒、発汗)は非特異的であるため、診断およびアトバクオンやアジスロマイシンによる有効な治療の遅れの一因となり、診断上の課題を呈している。ある研究では、症状発現から治療までの平均遅延は12~14日であった。バベシア症の発生率および地理的分布の知識は、臨床上の指標をもたらし、迅速な診断および救命治療を促進する。ウィスコンシン州でのバベシア症の初めての症例は1985年に発見され、2001年に正式に州に報告義務のある疾患となった。2001~2015年のウィスコンシン州のバベシア症調査データは3年間隔で分析され、患者間の人口統計、疫学、臨床検査の特徴を比較している。報告されたバベシア症増加の考えられる理由を究明するために、PCR法による電子臨床検査報告および診断の傾向を調査した。2001~2015年に計430例のバベシア症が報告され、このうち294例(68%)が確定例、136例(32%)が高度疑い例であった。ウィスコンシン州居住者10万人あたりのバベシア症確定例の平均年間発生率は、2001~2003年では0.03であったが、2004~2006年では0.15、2007~2009年では0.17、2010~2012年では0.57で、前期と比べそれぞれ400%、13%、235%増加した。2013~2015年の平均年間発生率は0.80で、2001~2003年と比べて26倍増加した。2001~2015年の間では、確定例が居住する郡は、ウィスコンシン州72郡のうち50郡(69%)におよんだ。確定例のうち163例(56%)が記録され、バベシア症との関連が推定される郡として、36郡およびその他の州(マサチューセッツ州)が該当した。これらの163例では、137例(84%)で患者の居住郡とダニの曝露が推定される郡が一致し、ウィスコンシン州の33郡(北西部:13郡、北東部:9郡、南部:11郡)で認められた。居住者間のバベシア症の1回以上の報告があった郡の数は2006~2010年では30郡、2011~2015年では46郡で、州の全ての郡の42%、64%に該当した。これらの報告されたバベシア症の地理的拡大は、主に初期に発生した地域の東部および南部に伸展した。これらの傾向は、他の州でも風土病、同様の郊外化および森林破砕パターン、平均気温の温暖化を伴い発生している可能性がある。バベシア症およびその他のダニ媒介性疾患の増加する負担を推定し、適切な公衆衛生介入を実施するためには、バベシア症が固有である州における正確な監視が必要である。
