Vital Signs: Increased Medicaid Prescriptions for Preexposure Prophylaxis Against HIV infection — New York, 2012–2015

バイタルサイン：HIV感染に対する曝露前予防薬のMedicaid処方の増加 ― ニューヨーク州、2012～2015年

2014年6月29日に、ニューヨーク州知事は州における伝染病としてのヒト免疫不全ウイルス(HIV)/後天性免疫不全症候群(AIDS)を終了するための計画を練り上げた。この取り組みの目標は、2020年末までに新規に診断される年間のHIV感染数を推定3,000人から750人に減らすことであり、その結果、州のHIV感染率は史上初めて減少する。この計画の主なポイントは、1)HIV未診断者の同定と治療への結び付け、2)ウイルス抑制の可能性を最大限に生かすために、HIV感染症と診断された人のヘルスケアへの結び付けと維持、 3)高リスクでHIV陰性者のウイルス感染および伝播を防止するために、曝露前予防薬(PrEP)へのアクセスを容易にすることである。今回、ニューヨーク州のMedicaid 受給者において、2012年7月～2015年6月のPrEPの処方数の変化を調査した。経口抗レトロウイルス薬ツルバダのPrEPとしての使用(＞30日)を同定するために、薬剤および診断コード使用したアルゴリズムにより、処方データからHIVの曝露後予防薬または治療および慢性B型肝炎感染への使用を除外した。1回以上のPrEPのための処方を受けたMedicaid 受給者の総人数は、2012年7月から2013年6月では259人、2013年7月から2014年6月では303人であったのに対し、2014年7月から2015年6月では1,330人となり、初年度と比べ338.9％増加した。全期間にわたり合計1,708人で1回以上のPrEPが処方され、大半がニューヨーク市の居住で、男性、年齢50歳未満、白人が多かった。州および地域の保健部門はPrEPの利用可能性および使用に寄与しているが、協調によりPrEP使用を増加させるための戦略の成功実現が増強すると考えられる。さらに、州のMedicaid機関によるPrEPを補償するという決定は、HIV感染予防の長期的な利点を承認している。
