Exposure to Elevated Carbon Monoxide Levels at an Indoor Ice Arena — Wisconsin, 2014
屋内アイスアリーナにおける高濃度の一酸化炭素への曝露 ― ウィスコンシン州、2014年
2014年12月13日、ウィスコンシン州レイクデルトンの緊急管理システムに、選手約50名、参加者約100名が集まる屋内ホッケートーナメントに出場していた20歳男性の選手が意識を失ったとの連絡が入った。消防による調査の結果、アリーナ内の一酸化炭素(CO)濃度が45~165ppmに上昇していたため、アリーナ内のすべての選手および参加者に対し、CO中毒の可能性を検査するよう呼びかけ、12月13~14日、計92名(男64名、女28名、0~15歳:9名、16~20歳:52名、21~25歳:9名、25歳以上:22名)が救急外来(ED)を受診した。症状は頭痛:84%、悪心:66%、嘔吐:26%、めまい:19%、息切れ:10%などを認め、77名(84%)から採取した静脈血中COHb濃度は0~21.7%(平均:9.7±4.4%)であった。全体で74名(80%、うちホッケー選手は32名)がCO中毒の診断基準に合致し、血中COHb濃度は5.1~21.7%(平均:11.1±3.6%)、ホッケー選手のCOHb濃度は平均13.0±3.6%と観客(9.7±3.0%)と比べて有意に高値であり、アリーナで意識を失った選手のCOHb濃度が21.7%と最も高かった。意識を失った選手と妊娠19週の女性(25歳)には入院にて高圧酸素療法が、91名に酸素補充療法が行われ、痛みに対しアセトアミノフェン(14名)、イブプロフェン(1名)、メペリジン(1名)、悪心、嘔吐、めまいに対しオンダンセトロン(18名)、プロメタジン(2名)が投与された。約2カ月後、91名に郵送にて調査を実施、47名(52%)から回答を得た。回答者のうち、CO中毒であった36名の症状はめまい(70%)、息切れ(33%)、胸部絞扼感(22%)などがあり、うち6名(17%)は記憶障害(4名)、集中力低下(4名)などの症状が7週間以上持続していた。また、他の持続症状として咳、息切れ、気分変動(各1名)などがあった。イベント当日の夜のCO濃度は45~165ppm、選手が意識を失ったロッカールームは140ppmであり、基準値(20ppm未満)を大きく上回っており、原因として、週末に使われる整氷車から排出されるCO[総排気量の4.8%(稼働時)、2.3%(アイドリング時)]の可能性が考えられている。
