Vital Signs: Multistate Foodborne Outbreaks — United States, 2010–2014

バイタルサイン：複数州における食品が媒介したアウトブレイク ― アメリカ、2010～2014年

CDCのFoodborne Disease Outbreak Surveillance Systemデータより、2010～2014年にアメリカで発生した複数州における食品媒介性疾患のアウトブレイクについて報告する。この5年間、CDCに報告された複数州における食品媒介性疾患のアウトブレイクは120件[年平均24(19～26)件]であり、関係した州は6(2～37)州/件、国内すべての州、ワシントンDCおよびプエルトリコにて発生を認め、症例数は22(2～1,939)例/件であった。件数はアメリカで発生した飲食に起因するアウトブレイク3％(120/4,163件)であったが、症例数は11％(7,929/71,747例)であり、入院例は34％(1,460/4,247例)、死亡例は56％(66/118例)を占めていた。起因菌はサルモネラ菌がもっとも多く(63件)、志賀毒素産生大腸菌(STEC)(34件)、リステリア菌(12件)と続き、サルモネラ菌は症例数の82％(6,530/7,929例)、入院例の65％(952/1,460例)の起因菌であり、もっとも規模の大きいアウトブレイクに関与していた(卵：1,939例、鶏肉：634例、生マグロ製品：425例)。また、サルモネラ菌の血清型はNewport(10件、16％)、Enteritidis(6件、10％)、Javiana(5件、8％)が多く認められた。STECによるアウトブレイクのうち、14件(41％)は野菜(葉菜類)、8件(24％)が牛肉によるものであり、20件(59％)は血清型O157を起因菌とした。リステリア菌によるアウトブレイクのうち、6件(50％)は乳製品、3件は果物、1件はスプラウトによるものであり、死亡例は57例(86％)と多く、カンタロープによるアウトブレイクでは33例が死亡している。また、輸入食品によるアウトブレイクは18件(15％、メキシコ：6件、トルコ：3件)であり、うち6件は果物、4件は木の実/種、魚および野菜によるアウトブレイクが各2件認められた。この期間、87件(73％)のアウトブレイクに関して原因となる製品の追跡調査が行われ、55件(46％)に関し製品のリコールが行われている。
