致命傷の生涯医療費および労働損失費の試算 ― アメリカ、2013年
2013年のアメリカにおいて、不慮の外傷、自殺、殺人はそれぞれ主な死因の4位、10位、16位を占め、これらの3つを合計すると約187,000例の死因となる。致命傷の経済的影響を評価するため、CDCはWeb-Based Injury Statistics Query and Reporting System データベースと2013年のNational Vital Statistics Systemを分析し、性別、年齢グループ、故意なのか不慮なのか、受傷機転により、死亡数、死亡率、致命傷に対する総生涯医療費および労働損失費を算出した。
2013年における致命的な傷害の割合は10万人あたり61.0、推定総生涯医療費および労働損失費は2,140億ドルで、男性はこれらの費用の78%および傷害死亡の67%を占めた。傷害死亡の約2/3は不慮によるもので費用の大部分(1,297億ドル、61%)を占め、約1/5は自殺で費用の24%(508億ドル)を占めた。殺人に関連した死亡は8%で、関連する費用は12%(264億ドル)であった。全体で、致命傷の割合は年齢とともに上昇したが、殺人および自殺ではそれぞれ青年および中年で上昇した。致命傷に関連する費用は、青年および若年成人(15~24歳)と成人(25~64歳)に集中しており、65歳以上では最も低下した。すべての傷害死亡において、薬物中毒は致命傷に関連する費用の26%を占め、次いで交通関連死(23%)、銃器関連死 (22%)、窒息死(9%)、転落死(5%)であった。不慮の傷害死亡に関連する費用は、主に交通関連死(37%)および薬物中毒(38%)であったが、殺人および自殺に関連する費用では、銃器による致命傷がそれぞれ75%および48%を占めた。これらのことから傷害関連の死亡は経済的な負担となっており、有効な予防策の必要性が示唆される。
