大学における血清群B型髄膜炎菌性疾患のアウトブレイクおよび保菌調査 ― ロードアイランド州、2015年

2015年2月2日、ロードアイランド州保健局はプロビデンス大学の学生における髄膜炎菌性疾患の発生報告を受けた。その3日後、同校の学生で初発例と接触のない2例目の発生が報告された。両例とも非致死例で、まれな流行株であるNeisseria meningitidis 血清群B型(ST-9069)が検出された。発生を受け、患者の気道分泌液に曝露した可能性のある71名に対し、シプロフロキサシンの予防的投与が行われた。また、同校生徒に対しては髄膜炎菌性疾患の徴候および症状と伝播予防のための衛生手順について教育が行われた。分離株の遺伝子型検査から、最近認可された2種類の血清群B型髄膜炎菌ワクチン(MenB-FHbpおよびMenB-4C)による交差防御が予想され、2月8日および11日にMenB-FHbpの集団接種が実施された。全在校生、キャンパス内に居住または勤務する25歳未満の卒業生およびスタッフ等、同校に関連する3,745名のうち3,525名(94％)が初回接種を完了し、6月8日現在、同校関連の追加症例は発生していない。2月16日～20日、鼻咽頭の髄膜炎菌(N. meningitidis)保菌率およびMenBの予防接種効果の調査を実施した。調査対象者717名のうち470名(66％)は女性、655名(91％)はキャンパス内に居住し、701名(98％)はMenB-FHbpの初回接種済みであった。予備的検査の結果、176名(25％)が髄膜炎菌を保有し、うち31名(4％)がB血清群、8名(1％)はC群、1名(1％未満)はX群、4名(1％)はY群、132名(18％)は群別不能であった。ST-9069は検出されなかった。男性、喫煙者、バーやナイトクラブ、パーティへの参加回数が週に1回以上といった因子が高保菌率と関連し、最近の抗生剤服用は低保菌率と関連していた。同校の学生における保菌率は、最近英国で報告された大学生の保菌率(34％)と同等である一方、アメリカの一般的な人口における推定保菌率1～8％よりも高かった。今回の調査では流行株は検出されなかったが、4月に第2回調査を実施、現在検査中であり、第3回調査は秋に実施を予定している。
