Vital Signs: Melanoma Incidence and Mortality Trends and Projections — United States, 1982–2030
バイタルサイン:メラノーマの発症率および死亡率の傾向と予測 ― アメリカ、1982~2030年
CDCは2011年のメラノーマの発症率および死亡率データを分析し、2030年におけるメラノーマの発症率、死亡率、新規診断例に対する医療費の予測を行った。アメリカではメラノーマの発症率が増加しており、2011年の症例数は65,647例、年齢補正発症率は10万人あたり19.7(男性:25.3、女性:15.6)と報告されている。人種/民族別では非ラテン系白人が最も高く(24.6)、15~49歳では発症率は女性の方が高く、50歳以上では男性の方が高かった。2011年におけるメラノーマによる死亡例は9,128例、年齢補正死亡率は10万人あたり2.7(男性:4.0、女性:1.7)であり、人種/民族別では非ラテン系白人(3.4)にて最も高く、年齢に伴い増加した。メラノーマ発症率は1982年に比べ約2倍に増加、死亡率は1982~2011年にてほぼ一定であった。今後、新しい治療がないとした場合、2030年の新規例は112,000例と推定され、また、包括的皮膚がん予防プログラムにより2020~2030年にてメラノーマの発症を20%抑制すると推定されるため、年平均21,000例の発症が回避される(2020~2030年にて計230,000例)。新しい治療介入がない場合、メラノーマ新規診断例の年間治療費は2011~2030年の間に252.4%増加し(4億5,700万ドルから16億ドル)、包括的皮膚がん予防プログラムによる年平均2億5,000万ドルの医療費が削減されると推定された(2020~2030年にて計27億ドル)。以上、新しい治療介入のない場合、メラノーマ新規診断例にかかる医療費は2030年までに3倍以上となるが、皮膚がん予防プログラムによる大きく削減できることから、紫外線曝露を避け、屋内での日焼けや日焼け止めの使用を増やすなどの対策のほか、皮膚がんの早期診断/治療の重要性が示唆された。
