エボラウイルスの性行為感染の可能性 ― リベリア、2015年
リベリアにおいて最後のエボラウイルス病(Ebola)の確診例が隔離された30日後の2015年3月20日、モンロビアにて1人の女性がEbolaであると検査にて確診された。この女性(44歳、患者A)は3月14日、頭痛、脱力感、関節痛、悪心を来し、19日に受診、20日にRT-PCR法にて確診された。Ebola流行国への渡航歴はなく、シエラレオネやギニアからの旅行者との接触、葬儀への参列、Ebola患者との接触もなかったが、3月7日、46歳のEbola生存者と避妊せずに性行為を行っていた。この男性は2014年9月9日、発熱、食欲不振、頭痛を来し、9月23日にEbola治療施設へ入院、28日のRT-PCR法では確診されず、10月3日の検査も陰性であった。10月7日に治療施設を退院し、その後、症状は認めていないが、この男性の家族も同じ時期に発症し、兄、弟、娘および息子はいずれも死亡している。2015年3月23日に採取した男性の血液のRT-PCRによる検査は陰性であったが、ELISA法ではエボラウイルス糖タンパク質および核タンパク特異免疫グロブリンG(IgG)抗体が陽性であり、IgMは検出されなかった。さらに3月27日に採取した男性の体液はRT-PCR陽性であり、エボラウイルスの遺伝情報が検出された。この男性は患者Aの他に45歳の女性とも性行為を行っていたが(2月末~3月15日の間に3~5回)、この女性から3月27日に採取した血液検体はエボラウイルス特異IgGおよびIgMともに陰性であった。以上より、Ebola生存者の体液におけるウイルスのクリアランスおよび持続性と性行為による感染の可能性に関する検討が必要であると思われる。
