Sustained Decrease in Laboratory Detection of Rotavirus after Implementation of Routine Vaccination — United States, 2000–2014
ワクチン定期接種実施後における検査でのロタウイルス検出の持続的な減少 ― アメリカ、2000~2014年
ロタウイルスは乳幼児に重度の胃腸炎をきたす主な原因菌であり、アメリカでは2006年にワクチンが導入される前はロタウイルス感染症により推定55,000~70,000例の小児が入院し、年間受診例は410,000例であった。また、ロタウイルス感染症は冬から春にかけて発生し、地域的には西部(12月~1月)から北東部(4月~5月)に広がっていた。今回、CDCはこれらの特徴について、National Respiratory and Enteric Virus Surveillance Systemのデータをワクチン導入前(2000~2006年)と導入後(2007~2014年)にて比較した。ワクチン導入前、発症開始は第50週(12月)、ピークは第9週(2月/3月、検体の43.1%が陽性)、発症を認めた期間は26週間であった。ワクチン導入後は発症開始が遅くなり(2007-08年:第9週、2008-09年:第4週、2010-11年:第3週、2012-13年:第1週、その他の年は閾値に達しなかった)、ピークは第11週~第22週、ピーク時の検体陽性率は10.9~27.3%と低下し、さらに発症を認めた期間も短縮した(0~18週間)。西部、南部、中西部および北東部における発症開始、ピークおよび発症期間はそれぞれにバラツキがあり、とくに南部では発症開始が第49週~第17週、ピークは第9週~第21週、発症期間は0~31週間と、その年により大きく異なっていた。なお、ワクチン接種率は19~36カ月齢の幼児にて2009年:43.9%、2013年:72.6%と増加しており、また、2007~2011年の4年間におけるロタウイルス感染症の入院は176,000例、救急外来の受診は242,000例であり、この4年間で下痢による外来受診者が約110万例回避され、約9億2400万ドルの医療費が削減された。
