CDC Grand Rounds: Preventing Youth Violence

アメリカでは2013年、10～24歳の若者4,481人(10万人あたり6.9)が殺人により死亡しており、外傷、自殺に次いで3番目に多い。男女別では男性：11.7、女性：2.0と男性にて約6倍多く、人種別では非ラテン系白人：2.1に比べ非ラテン系黒人：27.6(約13倍)、ラテン系：6.3(約3倍)にて多かった。また、暴力により負傷した若者は死亡した若者の約100倍であり、547,260人が暴行による非致死性外傷により救急外来を受診した(10万人あたり847)。CDCの2013 Youth Risk Behavior Surveyによると、昨年、約1/4の高校生が少なくとも1回以上、身体的ケンカをしており、17.9％が過去30日以内に学校へ武器(銃、ナイフ、こん棒)を持って行ったと報告している。また、昨年学校でいじめを受けた高校生は19.6％であり、6.9％は校内で武器により脅されたまたは傷つけられており、7.1％が学校または通学路での危険を感じ、登校しなかった。これら若者による殺人、非致死性の暴力による負傷による医療費と生産性損失は195億ドルと推定されている。若者の暴力は阻止することが可能であり、一般的に暴力に対する考え方や感情を変え、暴力を避けるスキルを養い、非暴力的に解決することを目的とする学校をベースとするプログラムが広く行われている。また、両親や家族に対するプログラムや地域レベルでの活動も進められており、CDCのStriving to Prevent Youth Violence Everywhere(STRYVE)は地域の力を増強し、様々な対策を共同して行っている。また、個人、関係者および地域レベルでの危険因子に対する包括的な予防プログラムであるCDCのYouth Violence prevention Centers(YVPCs)により、シカゴでは2010～2013年に殺人が50％減少したと報告されている。暴力の阻止により罹病率や外傷による死亡率が低下し、アルコールや物質中毒、肥満、慢性疾患などの健康に関する問題のリスクも低下すると考えられる。また、裁判や教育にかかる費用が削減されるため、公衆衛生の専門家による包括的な暴力対策の重要性が示唆される。
