エボラウイルス病生存者のコミュニティへの復帰 ― リベリア、ファイアストン地区、2014年

エボラウイルス病(Ebola)生存者のコミュニティへの復帰は、包括的Ebola対策の重要な要素の一つである。過去のEbolaアウトブレイクにおいては、生存者の35％が家族や友人、隣人から忌避されたと感じたと報告している。ファイアストン社はリベリア中央のファイアストン地区にある天然ゴム農園運営会社で、従業員とその家族、退職者および地区内の居住者約8万人に医療を提供している。2014年4月に同地区で初めてのEbola患者が発生して以降、会社はEbola治療施設(ETU)の提供を含む包括的Ebola対策を実施し、生存者の復帰プログラムを策定した。2014年8月1日～11月1日に、Ebola確診例のうち33例がファイアストンETUにて死亡し、22例(42％)が確診後に回復、再検査後に退院した。生存者の平均年齢は23歳と死亡例(38歳)に比べ有意に低かった。生存者のETU滞在期間は平均16日であった。生存者のコミュニティへの復帰計画はETUを離れる前から開始され、社の復帰支援チームが生存者の家族および近隣住人、コミュニティのリーダーに対し計画の説明および教育、生存者のEbolaフリー宣言を行った。復帰当日には、社の医療責任者およびETUコーディネーター等医療スタッフ、復帰支援チーム、ラジオ放送局員、聖職者が生存者を伴いコミュニティを訪問し、コミュニティのリーダーによる開会挨拶と復帰歓迎の表明、保健福祉省の代表者や生存者本人の談話、医療責任者からEbolaフリー証明書の進呈等の式典を行い、その様子をラジオで放送した。生存者には寝具、せっけん等衛生用品、米や油、衣類、現金といった生活必需品が支給され、復帰後は3カ月間、社の医療チームが毎週自宅を訪問し、健康状態の確認と社会心理学的支援を行った。従業員は仕事に復帰し、孤児には保護者を指名し通学できるようにした。現在まで大きな問題は発生せず、住居の問題や生存者への襲撃、コミュニティの動揺は認められていない。今後、同様の復帰プログラムの公的な実施により、噂や忌避の低減と感染者のより早期の医療機関受診が促進され、伝播の低減と生存率の向上が期待される。
