Clinical Inquiries Regarding Ebola Virus Disease Received by CDC — United States, July 9–November 15, 2014

CDCに寄せられたエボラウイルス病に関する臨床的問い合わせ ― アメリカ、2014年7月9日～2014年11月15日

2014年初めより主にギニア、リベリア、シエラレオネにてエボラウイルス病(Ebola)による死亡例が6,000例以上報告されている。CDCは2014年7月9日よりアウトブレイクに対応するためEmergency Operations Centerを始動させ、州および地方の公衆衛生局の職員からの問い合わせやEbolaのリスク評価をサポートしており、7月9日～11月15日の間に49州およびワシントンDCから650名に関する問い合わせがあった。人口統計学的情報の明らかな症例のうち、女性は49％、年齢中央値は34(9カ月～90)歳、18歳未満が16％であった。138例(21％)がエボラ感染国に旅行しており、22例(3％)はアメリカ国内でEbola患者または患者の体液と接触しており、490例(75％)にリスク因子は認めなかった。118例(74％)はEbolaの少なくとも1つの徴候または症状があり、PUI(要観察例)の基準を満たしていた。PUIに関する問い合わせは34州およびワシントンDCから寄せられており、件数は9月30日までは平均10件/週であったが、CDCがアメリカで初のエボラ患者を確認した後に問い合わせは急増し、10月中旬には227件に上った。エボラウイルスの検査は61例(9％)に行われ、うち35例(57％)はエボラ感染国へ旅行歴があり、16例(26％)はアメリカ国内でEbola患者または患者の体液と接触しており、10例(16％)にリスク因子は認めなかった。Ebolaの確診例は4例(うち1例が死亡)であり、3例は医療従事者、うち2例は初症例のケアを行っていた。二次感染は認めていない。2014年10月27日、CDCはエボラ感染国からの旅行者すべてを対象に、入国時スクリーニング後、最終エボラウイルス曝露から21日間、体温および症状のモニタリングを行っており、エボラ感染国に滞在していた17例のうち14例がPUIと判断され、9例にエボラウイルス検査が行われたが、陽性例は認めていない。
