Evidence for a Decrease in Transmission of Ebola Virus — Lofa County, Liberia, June 8–November 1, 2014

ロファ郡におけるエボラウイルス伝播減少のエビデンス ― リベリア、ロファ郡、2014年6月8日～11月1日

リベリアでは2014年10月31日までにエボラウイルス病(Ebola)の症例数が約6,500例に達した。人口約30万人のロファ郡はリベリア国内でEbola発生が最も多い地域の1つであり、10月末までの累積症例数は623例である。リベリアにおけるEbolaの初発例は、2014年3月にロファ郡内の人口約20,000人のフォヤ地区で発生した。発生に対応してただちに地域保健局、国境なき医師団(MSF)らNGO、ヨーロッパ移動検査室(EMLab)ら技術専門家による協力体制がとられた。リベリア保健社会福祉省(MoHSW)の最新の報告によれば、ロファ郡における新規症例数は減少が見られる。CDCはMoHSWの要請を受け、6月8日～11月1日の各症例データについて精査した。初発例発生後、4月9日～5月31日まで続発例の発生はなかったが、6月初旬からの新規症例数の週計は6月8～14日の週(12例)から8月10～16日の週(153例)まで増加、その後減少し、10月26日～11月1日の週は4例であった。MSFがフォヤ地区に設置したEbola治療施設(ETU)の受診者の個別データでは、最終的にEbolaを否定された症例は6月8日～8月9日には25％、8月10～16日の59％をピークとして漸減し、8月17日～11月1日は41％であった。総計で、フォヤ地区のETU受診者の40%はエボラウイルス以外を原因とする疾病であった。また、ETU以外のコミュニティで発生した死亡者の口腔スワブはEMLabにてRT-PCR検査を行った。その結果、6月8日～8月16日は35/37検体(95％)がエボラウイルス陽性であったが、8月24日～11月1日の陽性は21/85検体(25％)のみであった。以上のデータから、6月初旬のEbola再興に対応した緊急活動が功を奏し、8月17日以降、ロファ郡のエボラウイルス伝播は減少したと考えられた。ロファ郡における包括的な対策は、未だエボラウイルス伝播が継続する地域における対策のモデルとなるだろう。
