Advisory Committee on Immunization Practicesの歴史と発展 ― アメリカ、1964～2014年

Advisory Committee on Immunization Practices(ACIP)は、アメリカ一般市民のワクチン使用に関してCDCおよびアメリカ保健福祉省(DHHS)長官へ専門家が外部から助言する連邦諮問委員会として公認されている。1940年代～1950年代では、アメリカ公衆衛生局(USPHS)はさまざまな生物製剤に関連する問題を間欠的に招集される委員会に諮っていたが、1960年代前半までに付加的な新規のワクチン(1961年の単味経口ポリオワクチン、1963年の三価経口ポリオワクチンおよび麻疹ワクチン)の免許下付およびワクチンおよび予防接種の資源への連邦政府の投資の増加に伴い、国家的な免疫政策の取り組みのために特別委員会の形成よりも、より継続的に専門家の技術的な助言を提供する委員会が必要となった。ACIPはUSPHSの長官により1964年に創設され、初回の委員会の議事録には、委員会は防疫に使用される可能性のある特異的な予防薬の公衆衛生業務での最も効果的な適用に関して、長官へ助言する責任があることが記載された。委員会により考慮される薬剤は、不活性化および弱毒化した細菌/リケッチア/ウイルス因子、毒素、抗毒素、化学予防薬、免疫グロブリンが含まれ、委員会はこれらの薬剤の使用のための予防接種スケジュール、用量および投与経路、適応と禁忌の設定に関与し、薬剤が利用できる人口集団の相対的優先順位、集団予防接種実施の優劣および方法、研究で必要とされるプログラムについて、適切に助言しなければならない。ACIPは1972年に連邦諮問委員会に指定され、委員数は当初の8人から投票権のある委員は15人まで拡大している。ACIPの設立から50年で、小児/青年(0～18歳)の予防接種スケジュールにおいて、予防接種により予防可能な疾患は6種類から16種類に増加し、日常的に推奨されるワクチン数も増加している。1995年には小児/青年の予防接種スケジュールが、初めてACIP、American Academy of Family Physicians、American Academy of Pediatricsにより協調して承認され、現在も専門学会との協調が継続している。ACIPは当初の役割に加え、小児に対するワクチンプログラムの推進、ワクチン事業の経済的分析を実施し、エビデンスのレビュー過程の透明性および品質を強化するためにGrading of Recommendations Assessment, Development and Evaluationシステムを導入している。ACIPは、消費者の態度および地域の価値観を取り込むための最善の方法をはじめとする困難な課題に直面しており、また、小児/青年の予防接種スケジュールで推奨された増え続けるワクチン数の勧告と調整に対する経済的分析に関する困難な審議も実施している。
