Typhoid Fever Surveillance and Vaccine Use — South-East Asia and Western Pacific Regions, 2009–2013

腸チフスのサーベイランスとワクチンの利用 ― 東南アジアおよび西太平洋地域、2009～2013年

腸チフスは主にアジアにて認められる重篤な全身性感染症であり、症例数は年間約2,200万、死亡数は216,500例である。WHOは2008年、腸チフスのワクチン接種およびサーベイランスの強化に関する勧告を出しており、今回、2009～2013年のWHO の東南アジア地域(SEAR)および西太平洋地域(WPR)におけるワクチン接種プログラムおよびサーベイランスの現状に関する報告がまとめられた。SEAR(11)およびWPR(37)の計48の国および地域のうち、23カ所(48％)にて症例のデータが収集され、うち22カ所では腸チフスが法定伝染病に指定され、20カ所にてサーベイランスが行われていた。標準症例基準は15カ国にて定められているが、その基準は国により異なっており、また、臨床検査は19カ国にて行われ、17カ国では血液培養(17/19)、便培養(15/19)にて確診、10カ国ではWidal反応、1カ国は他の迅速検査を使用していた。ワクチン接種プログラムは9カ所(19％)にて行われ、ワクチン接種の対象はハイリスク群および/または食品取扱者であった。さらに11カ国(オーストラリア、カンボジア、フィジー、インド、インドネシア、ネパール、ニュージーランド、フィリピン、シンガポール、スリランカ、タイ)では民間機関にてワクチンが使用されていた。中国、インド、ベトナムでは2008年よりも前から未就学児童および学童期の児童を対象としたワクチンプログラムが実施されていた。以上、SEAR、WPRでは腸チフスのサーベイランスおよびワクチン接種のプログラムは遅れており、診断のガイダンス、治療方法に関しても整備の必要性が示唆された。
