Vaccination with Tetanus, Diphtheria, and Acellular Pertussis Vaccine of Pregnant Women Enrolled in Medicaid — Michigan, 2011–2013

2011年10月に、Advisory Committee on Immunization Practicesは百日咳から乳児を守るための戦略として、妊娠中の破傷風、ジフテリア、無細胞百日咳ワクチン(Tdap)の定期接種を初めて推奨した。この推奨は過去にTdap未接種の女性が対象で、ワクチン接種の最適な時期は妊娠第2期または第3期(妊娠期間20週後)である。妊婦のワクチン接種により、百日咳に高リスクの乳児は子宮内で母体由来抗体から受動免疫を得ることができる。ミシガン州では、メディケイドが出生の約40％の妊娠費用を負担しているが、メディケイドに登録されている乳児は個人の保険に加入している乳児よりも全原因死亡率が高く、影響を受けやすい集団である。このため、ミシガン州の公的資金を受けた保険プログラムに登録された妊婦間の予防接種率を調査した。2011年11月～2013年2月に出生した乳児の母親について、メディケイドの診療報酬請求および州全体の予防接種情報システムデータを分析した。対象となったのは15,181例(出産時の年齢の中央値：21歳)で、白人は59.3％、黒人は29.6％であった。約65.5％の乳児は満期(39週以上)で出生した。対象集団の半数以上が、中等度(37.6％)または不適切(37.5％)の妊婦健診を受けていた。対象集団のうち、妊娠中にTdap接種を受けたのは14.3％で、人種/民族における接種率は、白人：17.6％、黒人：8.4％、アジア系：11.9％、ネイティブアメリカン：21.9％、ヒスパニック系：15.3％、アラブ系：6.8％であった。出産時の母体の年齢および在胎期間は、Tdap接種の有意な予測因子であったが、妊婦健診の適切さは関連しなかった。すべての乳児が母体の予防接種による百日咳の予防を受けられるように、医師、両親、家族への啓蒙のような努力を強化する必要がある。
