Use of MenACWY-CRM Vaccine in Children Aged 2 Through 23 Months at Increased Risk for Meningococcal Disease: Recommendations of the Advisory Committee on Immunization Practices, 2013

髄膜炎菌感染症リスクが高い2～23カ月児に対するMenACWY-CRMワクチンの使用：Advisory Committee on Immunization Practicesの勧告、2013年

2013年10月のAdvisory Committee on Immunization Practicesの定例会議にて、髄膜炎菌感染症リスクの高い2～23カ月齢の小児に対する3種類目のワクチンとしてMenACWY-CRMが追加された。MenACWY-CRMは2～8カ月齢の小児を対象に認可された初めての4価髄膜炎菌(血清群A、C、W、Y)-ジフテリア毒素結合型ワクチンである。既承認のワクチンMenACWY-Dは9～23カ月齢、Hib-MenCY-TTは6週～18カ月齢の高リスク小児を対象としている。2～23カ月齢の小児に対するMenACWY-CRMの免疫原性を評価した臨床試験では、2、6、12カ月齢で接種した場合、7カ月齢で血清群Aに対し74％、血清群C、W、Yに対し94％以上の防御抗体価が得られた。13カ月齢では、4血清群に対し94％以上の防御抗体が得られた。7価肺炎球菌ワクチンと同時接種した場合に、肺炎球菌血清型6Bおよび23Fに対する免疫応答の低下が見られたが、12カ月齢での同時接種では干渉は認められなかった。他の肺炎球菌血清型や定期接種ワクチン[DTaP-IPV-Hib、B型肝炎、麻疹・おたふくかぜ・風疹(MMR)、MMR+水痘]に対する干渉は認められなかった。約5,000例を対象に接種後6カ月まで追跡した5回の臨床試験において、有害事象の発現率は他の定期予防接種と同程度であり、死亡例の報告はなかった。髄膜炎菌感染症の高リスク小児として、持続的補体成分欠損症(C3、C5～C9、プロパージン、D因子、H因子)、鎌状赤血球症を含む機能的または解剖学的無脾症、髄膜炎菌感染症が発生中の集団に属しワクチン接種が勧奨される健康小児、髄膜炎菌感染症の高度流行地域または常在地域に居住あるいは旅行する小児が対象となる。高リスク小児に対するMenACWY-CRMの第1期接種は2、4、6、12カ月齢の4回接種とする。7～23カ月に接種を開始する場合は2回接種とし、2回目は初回接種から3カ月以上の間隔をあけて12カ月齢以降に接種する。髄膜炎菌血清群AおよびWに対する免疫抗原を含み、9カ月未満児を対象に認可されたワクチンはMenACWY-CRMのみであるため、髄膜炎菌感染症高リスク地域に旅行または居住予定の9カ月未満の小児は出発前にMenACWY-CRMを接種すべきである。その他のMenACWY-DおよびHib-MenCY-TTワクチンの使用に関しては従前の通りである。高リスク状態が継続する小児に対しては、追加接種として第1期終了後3年、その後は5年ごとの追加接種を推奨する。
