Trends in Tuberculosis — United States, 2013

CDCに報告された2013年の暫定的な結核(TB)サーベイランスデータに関して要約する。2013年、アメリカにおいて報告されたTBは9,588例であり、発症率は10万人あたり3.0と前年比4.2％の減少であった。州別の発症率(10万人あたり)は、ワイオミング州:0～アラスカ州:9.7 (中央値:2.2)であり、2012年に比べ33州にて減少、9州およびワシントンDCでは全体の3.0より高く、カリフォルニア州、テキサス州、ニューヨーク州およびフロリダ州では500例以上の発症を認め、この4州で計4,917例(全体の51.3％)であった。出生地別ではアメリカ生まれ:3,377例(35.4％)、国外生まれ:6,172例(64.6％)であり、症例数はともに2012年、2000年に比べ減少し(2012年比:アメリカ7.6％減、国外1.6％減、2000年比:各61.0減、19.0％減)、国外の出生地はメキシコ(20.0％)、フィリピン(12.6％)、インド(8.0％)、ベトナム(7.4％)、中国(6.1％)が多かった。発症率は全ての人種/民族別にて低下したが(白人:9.2％、黒人:7.5％、ヒスパニック系:5.3％、アジア系:0.3％)、アジア系にて最も高く、白人の25.9倍であった。HIV感染例は6.8％(2012年:7.4％)に認められ、多剤耐性結核は2012年86例であった。以上、アメリカでは結核の発症は全体的には低下していたが、今後もTBコントロールを必要とする特定の集団および地理的な相違が認められた。
