麻疹の世界的コントロールおよび地域的根絶、2000～2012年

2010年、World Health Assemblyは麻疹の世界的根絶に向け、2015年までに、1)1歳児におけるMCV1の初回接種率を国全体で90％以上、全ての地域にて80％以上とする、2)麻疹の年間発症率を100万人あたり5例未満とする、3)麻疹による死亡率を2000年の推定値から95％低下させると発表した。1歳児におけるMCV1接種率は、2000～2009年の間に73％から84％に増加し、2012年まで84％を維持している。90％を超える国は2000年にて83ヵ国(43％)から128ヵ国(66％)に増加した。MCV1未接種児は2012年にて約2,120万人と推定され、インド(640万人)、ナイジェリア(380万人)、エチオピア(100万人)、インドネシア(90万人)、パキスタン(70万人)、コンゴ共和国(70万人)にて多かった。2004～2012年、症例ベースのサーベイランスは120ヵ国(62％)から187ヵ国(96％)に増加して行われており、WHOのMeasles and Rubella Laboratory Networkへのアクセスは71ヵ国(37％)から191ヵ国(98％)に増加した。症例数は2000年:853,480例から2012年:226,722例と73%減少し、年間発症率は100万人あたり146から33へと低下しており、100万人あたり5例未満の基準を満たす国は2011年:104/188(55％)から2012年:119/187(64％)に増加している。また、麻疹による死亡者数は2000年:562,400から2012年:122,000例と78％減少し、死亡率も各地域にて低下していた[東地中海地域(EMR):52％～アフリカ地域(AFR):88％]。
