公衆衛生当局への電子的な検査結果報告の増加に関する進展 ― アメリカ、2013年

公衆衛生当局への検査結果の電子報告の増加は、報告をタイムリーおよび完全に作成することにより、報告義務のある疾患および状態に対する公衆衛生の調査を改善できる。2010年から、CDCは、臨床および公衆衛生臨床検査からの公衆衛生当局への電子検査報告(ELR)の改善を支援するために共同契約したEpidemiology and Laboratory Capacity for Infectious Diseasesを通じ、57の州、地方、地域の保健部門へ資金を提供している。合意の一部として、CDC、州、大規模な地方保健部門は、各管轄地域からの形成されたデータにより、アメリカにおけるELR実施のモニタリングを共同で取り組んでいる。ELRの進展のモニタリングは、Clinical Laboratory Improvement Amendments データベースおよびAmerican Hospital Associationからの2010年のデータを基に、各管轄地域の検査室リストの構築とともに2012年に開始した。これまでに、公衆衛生当局へ検査結果を送付した検査室は約10,400ヶ所で、内訳は、病院の検査室:約5,320(51％)、4つの大規模な民間試験所の施設:420(4％)、保健所:400(4％)、その他:4260(41％)であった。10,400ヶ所のうち、約5400ヶ所が保健部門によりELR導入の優先目標とされた。ELRにより報告された検査報告数を、全体の検査報告数と比較した。2013年7月31日現在で、57の管轄区域のうち、54の地域(48州および6つの大規模な地方保健部門)でELRを介して検査報告を受けた。2900の検査室(目標検査室の52%)が、1ヶ所以上の公衆衛生当局へELRを介して報告した。54の管轄区域から提供された12ヶ月の推定値に基づいて、全体検査報告の約62％が電子的に受理された。電子的に受理された検査報告の割合は、管轄区域により異なり、75％超は14地域、25％未満は9地域であった。電子的に受理したすべての報告のうち、40％は4つの大規模な民間試験所の一つからで、他は、病院の検査室:14％、保健所:30％であった。また、電子的に受理された検査報告の割合は疾患のカテゴリーにより異なり、一般的な伝染病では76％であったのに対し、HIVおよび性感染症は、これらの全体の報告量が多いにもかかわらず、54％と63％に低下した。電子的な検査報告の進展は、保健部門の技術的および政策的取り組みに対する能力および準備計画を増強および改善するために重要であり、臨床検査室、実験室情報システムのベンダー、公衆衛生当局間の協力が必要である。
