Deaths and Severe Adverse Events Associated with Anesthesia-Assisted Rapid Opioid Detoxification — New York City, 2012
麻酔下における急速オピオイド解毒に関連する死亡および重度の有害事象 ― ニューヨーク市、2012年
2012年8~9月、ニューヨーク市のPoison Control CenterよりDepartment of Health and Mental Hygiene (DOHMH)へ、麻酔下の急速オピオイド解毒(AAROD)により重篤な有害事象を来した3例(うち1例死亡)について報告された。DOHMHによる調査の結果、2012年1~9月、この施設(ニューヨーク市内)にて75例がAARODを受け、報告された症例を含め計7例(うち2例死亡)が72時間以内に入院を要する重篤な有害事象を来していた。AARODは1)禁断症状緩和のため、クロニジン、制吐剤、止瀉薬などを投与、2)挿管および全身麻酔導入、3)高用量のオピオイド拮抗薬ナロキソンを静注またはナルトレキソンを筋注、4)禁断症状の鎮静まで麻酔を維持、5)抜管し、その後回復までモニタリングという手順にて行われる。今回の調査での麻酔時間は8.3(3.1~15.0)時間、オピオイド拮抗薬投与時間:3.9(2.1~14.0)時間、ナロキソン投与量:80(2~315)mg、ナルトレキソン投与量:133(25~300)mgであり、重篤な有害事象を来した症例ではナロキソン投与量:80(4~88)mg、ナルトレキソン:150(0~150)mgであった。この施設でAARODを受けた75例は男性62例(83%)、年齢37(20~63)歳、合併症は精神障害(55%)、慢性疾患(23%)、薬物乱用(35%)であり、重篤な有害事象を来した7例は全例男性、31(24~52)歳、4例がオピオイド、2例はオピオイドとヘロイン、1例はヘロインを使用していた。また、4例が精神病を合併、2例が薬物乱用であった。オピオイド依存症の治療はエビデンスに基づき、死亡のリスクを考慮し、安全な方法で行われるべきである。
