Update to Interim Guidance for Preexposure Prophylaxis (PrEP) for the Prevention of HIV Infection: PrEP for Injecting Drug Users

HIV感染に対する暴露前予防薬(PrEP)暫定指針の改訂：注射薬物使用者におけるPrEP

米国では毎年約5万人が新規にHIV感染者となるが、2010年はそのうち8%が注射薬物使用による感染であった。2009年のThe National HIV Behavioral Surveillance Systemによる米国20都市の注射薬物使用者(IDU)の調査では、HIV感染に関し注射薬物使用と性行為とは同程度にリスクが高かった。経口テノホビルジソプロキシルフマル酸塩(TDF)はHIV感染症の抗レトロウイルス薬であり、曝露前予防薬(PrEP)としてHIV感染リスクを低減する。2005～2012年にタイ国で実施されたThe Bangkok Tenofovir Studyは、HIV非感染の注射薬物使用経験者を対象にTDFの安全性とHIV感染リスク低減効果を検証する第Ⅲ相無作為化二重盲検プラセボ対照試験である。20～60歳の2,413例をTDF 300mg/日投与群(TDF群: 1,204例)あるいはプラセボ投与群(1,209例)に割り付け、28日毎に服薬遵守状況、リスク低減カウンセリング、HIV検査、女性の妊娠検査、有害事象の評価を行った。また、試験実施機関では社会福祉サービス、プライマリ・ケア、メタドン、コンドーム、注射器の消毒が無償で提供された。フォローアップ期間は平均4.6年(最長6.9年)で、平均して服薬日の83.8%、対面検査日の86.9%が遵守された。試験登録後50例(TDF群: 17例、プラセボ群: 33例)がHIVに感染し、modified intent-to-treat解析ではTDF群における48.9%の、血中TDF検出によるunmatched case-control studyでは70%のHIV感染率低下が認められた。12ヶ月後のフォローアップ時には両群で同様にリスク行動の有意な低減［薬物注射(62.7%から22.7%)、注射針の共有(18.1%から2.3%)、複数のパートナーとの性交渉(21.7%から11.0%)］が認められ、試験期間中継続した。CDCはこれらの結果から、男性同性愛者や異性愛男女に加えIDUにおいてもTDFが有効であるとし、HIV感染予防の選択肢の一つとしてPrEP使用を推奨する。
