麻疹排除に向けた進展－西太平洋地域、2009～2012年

2005年に、WHOの西太平洋地域委員会は2012年までに麻疹を排除することを決断した。西太平洋地域において推奨される麻疹排除の戦略には、1)定期的な予防接種事業および必要であれば補足的な予防接種活動により、麻疹含有ワクチン(MCV)の2回接種の補償の達成と維持(95%以上)、2)高品質で症例に基づいた麻疹調査の実施、3)疑似患者の確認または除外および麻疹ウィルスの遺伝子型/分子分析のために、検体をタイムリーで正確に試験する技術を持った高品質の臨床検査による調査の確立、4)迅速な対応および適切な症例管理を確立するために、麻疹大発生への準備の構築と維持が挙げられる。37の西太平洋地域の加盟国および地域において、36のMCV補償に関する年間報告があった。2009～2012年の西太平洋地域では、1回目MCV接種(MCV1)の補償されている割合は96%から98%に増加し、定期的な2回目のMCV接種(MCV2)が提供されている割合は89%から92%に増加した。また、疑似症例の非麻疹としての除去率の目標を達成しデータが適切であった割合(50%から64%)、適切な調査を行った疑似症例の割合(38%から89%)、臨床検査のための適切な検体採取を行った疑似症例の割合(79%から93%)、7日以内に臨床検査結果が得られた割合(55%から96%)はそれぞれ増加した。2009年から2012年に確認された麻疹症例および人口百万人あたりの発生率は、それぞれ84%(54,291例から8,524例)、83%(34.0から5.9/百万人)減少した。2012年において、マレーシア(63.7/百万人)、フィリピン(15.9/百万人)、ニュージーランド(12.3/百万人)は、麻疹発生率が高率であった。中国の麻疹症例数は2009年(52,461例)には最大であったが2012年(6,183例)には88%減少した。2009～2012年に優勢であった麻疹ウィルスの遺伝子型は、H1:中国、D9:フィリピン、マレーシア、シンガポール、D8:マレーシアであった。西太平洋地域において麻疹消滅は達成されてないため、さらに1)MCV2の定期予防接種が行われていない4つの国および地域への定期的なMCV2の導入、2)MCV1またはMCV2の定期予防接種の補償が95%未満の国および地域における定期予防接種事業の強化、3)現在麻疹ウィルスが伝染している国および地域における感受性集団での補足的な予防接種活動による感染リスク期間の解消の努力が必要である。
