ワクチン未接種思春期健常児の水痘による死亡－オハイオ州、2009年

アメリカでは1995年に水痘ワクチンが導入され、水痘の発症、入院および死亡率は減少しているが、オハイオ州においてワクチン未接種の思春期の健常児が水痘により死亡した。これは2012年、Ohio Department of Healthによる人口動態統計の定期的調査において、2009年の水痘による死亡例を確認したものであり、国への報告義務のため、この症例に関する調査が行われた。2009年3月12日、思春期の女児が水痘による発疹、発熱および息切れのため入院、入院時の体温は38.4℃、呼吸困難、顔面浮腫、全身性点状出血性発疹および低血圧を来しており、敗血症性ショックと診断された。その後、呼吸器機能が低下し、非侵襲的人工呼吸器を必要とした。血液検査では血小板減少(血小板数:30,000/μL)、白血球減少(白血球数:1,400/μL)、血液培養は陰性であり、3月14日に採取した皮膚生検では水痘帯状疱疹ウイルス(VZV)陽性であった。入院中に肺炎を発症、急性呼吸窮迫症候群、汎血球減少症、多臓器障害、感染症(呼吸器および尿管感染症)および敗血症を来し、アシクロビル静注、抗生物質(シプロフロキサシン、メロペネム、トリメトプリム-スルファメトキサゾール、チカルシリン-クラブラン酸、チゲサイクリン)が投与されたが呼吸機能障害は進行し、入院21日目に死亡した。感染源は特定されず、また、予防接種についてはDPT(4回)、Hib(1回)、MMR(2回)を接種していたが、水痘ワクチン接種歴はなかった。以上、水痘ワクチン未接種の健常思春期女児の死亡が確認された。
