Cervical Cancer Screening Among Women Aged 18-30 Years － United States, 2000-2010

18～30歳の女性における子宮頸癌スクリーニング－アメリカ、2000～2010年

2012年、American College of Obstetricians and Gynecologists(ACOG)、American Cancer Society(ACS)およびU.S. Preventive Services Task Force(USPSTF)は、子宮頸癌のパップテスト(子宮頸部細胞診)を性行為の有無にかかわらず21歳未満の女性に対しては行わないこと、21～30歳の女性に対しては3年おきに実施することを推奨するガイドラインを発表した。CDCはこのガイドラインの導入前のパップテストの実施状況について、2000～2010年の18～30歳女性を対象としたBehavioral Risk Factor Surveillance System(BRFSS)データの分析を行った。分析対象データは50州およびワシントンDCより125,297名分が収集された。その結果、18～21歳の女性では、一度も検査を受けていない人の割合が2000年:26.3%から2010年:47.5%に増加し、過去12ヵ月以内に検査を受けた人は2000年:65.0%から2010年:41.5%に減少しており、全ての人種/民族にてこの傾向が認められた。22～30歳の女性では過去12ヵ月以内に検査を受けた人が2000年:78.1%から2010年:67.0%に減少し、過去13～24ヵ月および25～36ヵ月に検査を受けた人はそれぞれ9.8%から15.3%、2.6%から4.5%に増加しており、全ての人種/民族にて13～36ヵ月前に検査を受けた人が増加した傾向が認められた。また、一度も検査を受けていない人は2000年:6.6%から2010年:9.0%に減少していた。今回の調査はHPVワクチン接種の有無に関する考慮はしていないため、今後、スクリーニング検査の実施状況とワクチンの接種状況を合わせた調査が必要であると考える。
