Update on Vaccine-Derived Polioviruses － Worldwide, April 2011-June 2012　

ワクチン由来ポリオウイルスに関する最新情報－全世界、2011年4月～2012年6月

1988年、World Health Assemblyは世界的なポリオ根絶を発表したが、経口生ワクチン(OPV)接種率の低い地域や免疫不全患者ではワクチン由来のポリオウイルス(VDPV)によるアウトブレイクが発生している。今回、2011年4月～2012年6月に新たに認められたVDPVについて報告する。ポリオウイルスは3種類の血清型(PV1、PV2、PV3)があり、また、地域にてヒトからヒトへの感染のエビデンスを認める循環型(cVDPV)、VDPVに感染した原発性(B細胞)免疫不全患者から分離された免疫不全型(iVDPV)、感染源が不明の患者から分離された不明型(aVDPV)に分類される。2009年7月～2011年6月、常在性cVDPVは6ヵ国にて認められ、コンゴ共和国(DRC)、ソマリアではアウトブレイクが継続中、ナイジェリアでの大規模アウトブレイクは軽減し、モザンビーク、イエメンでは新しいアウトブレイクが発生し、マダガスカルでは再発したVDPVによるアウトブレイクが発生している。1961年のOPV導入以降、原発性免疫不全患者65名からiVDPVが排泄され、低開発国および中所得国にてiVDPV感染が2008年1月～2009年6月:2例、2009年7月～2011月:9例、2011年4月～2012年6月:12例認められている。また、2011年4月～2012年6月においてaVDPVが12ヵ国にて確認されている。VDPVの再出現および拡散の防止には、サーベイランスの強化とともにすべての国々がワクチン接種率を高く維持することが重要であり、また、3種類すべての血清型に対する免疫を維持するための不活化ポリオウイルスワクチンの使用を広め、アウトブレイクに対するワクチンの備蓄も重要である。
