Unexplained Respiratory Disease Outbreak作業部会の活動－世界中、2007年3月～2011年9月

Unexplained Respiratory Disease Outbreak(URDO)作業部会は、2004年に設立された感染性/非感染性呼吸器疾患の高度な専門知識を有する約40名の科学者から成るCDCの学際的チームであり、地域/州/全国の公衆衛生局による原因不明の呼吸器疾患アウトブレイクの調査をサポートしている。この報告は、2007年3月～2011年9月のURDO作業部会の活動をまとめたものである。この期間中、URDO作業部会は57件の調査に関与し、対象分野の専門家への相談(57件全て)やCDCでの検査(42件)を促進し、現場でのフィールド調査(8件)を支援した。57回の調査のうち41回は国内の事例、16回は国際的事例であった。正確な患者数が明らかになったアウトブレイク49件において、患者数中央値は15例(1～409例)であった。病因は29件(51%)で同定され、最も頻度が高かった病原物質はインフルエンザウイルス以外の呼吸器ウイルス(41%)、インフルエンザウイルス(17%)、Mycoplasma pneumonia(14%)、Bordetella pertussis(14%)であった。最初の患者の発症からURDO作業部会への通知までの期間中央値は33日(4～218日)と長く、発生早期の検査用検体の採取や疫学データの収集に影響を及ぼした。検体採取・疫学的調査・URDO作業部会への相談の遅延を少なくすることは、原因不明の呼吸器疾患アウトブレイクの病因を同定しより迅速にコントロールする能力を高める可能性がある。
