家庭内での曝露によるヒトOrfウイルス感染症－アメリカ、2009～2011年

orf(伝染性膿疱性皮膚炎)は一般に羊やヤギに感染するパラポックスウイルスによって起こる人獣共通伝染病であり、ヒトでは細菌感染症や悪性腫瘍に似た潰瘍性皮膚病変として発症する場合がある。ヒトのorfウイルス感染症は職業上動物と接触する人々や体験型動物園などに行った小児などで報告されているが、このような曝露歴のない患者は誤診され、不要な治療を受ける可能性がある。この報告は、2009～2011年に家庭内での肉の調理や宗教行事としての動物の屠殺に関連してorfを発症した4症例の臨床経過を述べたものである。63歳女性は最近屠殺されたヤギの骨が右手の掌にささり、その後損傷部位に丘疹が発現し、さらに水疱が形成された。42歳男性は宗教行事として子羊の屠殺を補助し、5日後に左手の第5指に水疱病変が発現し徐々に腫脹し疼痛性となった。35歳男性はイースターフェスティバルでの子羊の屠殺時に左手の親指をナイフで切り、その後同部位が腫脹し疼痛が発現した。28歳の妊婦は自宅で子羊の頭を調理中に骨で右手の掌を切り、同部位に水疱性病変が発現した。これら4例中2例は細菌感染症の疑いで抗生物質を処方されたが、症状は改善しなかった。転帰不明の1例を除く3例は病変部の外科的切除などを行い、orfは治癒した。全例の病変部検体は、定量的PCRにてorfウイルスDNA陽性であった。orfに特徴的な皮膚病変を有し、家庭内での肉の調理や動物の屠殺の経験のある患者の鑑別診断には、orfを考慮するべきである。またorfウイルスの曝露リスクを有する人々やコミュニティーは、感染予防のため手指衛生や非透過性手袋の使用などに関するカウンセリングを受ける必要がある。
