Surveillance for Chronic Hepatitis B Virus Infection － New York City, June 2008-November 2009

慢性B型肝炎ウイルス感染症－ニューヨーク市、2008年6月～2009年11月

アメリカにおける慢性B型肝炎ウイルス(HBV)感染症の罹患率は0.3～0.5%と推定されており、ニューヨーク市(NYC)のDepartment of Health and Hygiene(DOHMH)では毎年約11,500～13,000名が新たにHBV検査にて陽性が確認されている。DOHMHでは慢性HBV患者調査のため、新規診断例を対象にサーベイランスを行っており、今回は2008年6月～2009年11月に報告されたHBV感染患者から180例を無作為に抽出し、調査した結果を示す。180例のうちアンケート調査を完了した156例[38(2～91)歳、男:95名、女:61名、アジア系:67%]を分析対象とした。感染の原因は110名(70%)が不明であったが、周産期曝露(11名)、異性間性交渉(11名)、家庭内接触(10名)、同性間性交渉(6名)などが認められた。検査を受けた理由は出生地の有病率が高い(27%)、定期的なスクリーニング(19%)、危険因子がある(18%)、肝機能検査値が高い(12%)、肝炎の徴候や症状が認められた(2%)などであった。また、伝染や予防についてカウンセリングを受けた症例は75%、密接な接触者に対する告知についてのカウンセリングは69%が受けていた。サンフランシスコやミネソタ州での調査においてもアジア人の患者が多いことから、アジア系アメリカ人に対しHBVスクリーニングが推奨される。
