Measles － United States, January-May 20, 2011

アメリカでは1990年代後半以降、麻疹の継続的な伝播は認められていないが、2001～2008年には年間56(37～140)例の麻疹症例がCDCに報告されており、2011年は第19週までに118例が23州およびニューヨーク市より報告されている。これは1996年以降最も多い症例数である。症例は3ヶ月齢～68歳であり、年齢分布は12ヶ月齢未満:18例(15%)、1～4歳:24例(20%)、5～19歳:23例(19%)、20歳以上:53例(45%)であった。研究室レベルでの確診例は105例(89%)であり、麻疹ウイルスRNAは52例(44%)にて検出されている。118例のうち105例は輸入関連例であり、46例(44%)は少なくとも15ヶ国からの輸入例であり、ヨーロッパ地域:20例(うちフランス:11)、東南アジア地域:20例(うちインド:14)が多く認められた。入院例は47例(40%)、9例が肺炎を来したが、脳炎の発症はなく、死亡例も認めなかった。ワクチン未接種例は105例(89%)であり、1～19歳のアメリカ人患者45例中38例(87%)、20歳以上のアメリカ人患者42例中35例(83%)はワクチン未接種であった。また、海外旅行者33例のうち30例が未接種であったことから、麻疹の予防におけるワクチン接種の重要性が示唆された。
