Assessment of Declines in Reported Tuberculosis Cases － Georgia and Pennsylvania, 2009

結核患者報告数の減少の評価－ジョージア州とペンシルベニア州、2009年

アメリカにおける2009年の結核発症率(10万人あたり)は3.8例であり、1年間の低下率は11.4%と全国結核サーベイランスを開始した1953年以降で最大となった(2000年以降の年間低下率:平均3.8%)。2008年から2009年にかけて結核発症率が低下した36州のうち、ジョージア州では発症率が4.9例から4.2例へ14.3%低下し、ペンシルベニア州では3.1例から1.9例へ38.7%低下した。結核患者が同定されていない可能性を考慮し、Georgia Division of Public HealthとPennsylvania Department of HealthはCDCと協力して発症率低下の原因となる可能性のある4項目(サーベイランス関連因子、過小報告、未診断、実際の低下)について調査した。調査の結果、結核発症率の低下に影響を及ぼすほど重大なサーベイランス関連因子(報告や調査の方法の変化・遅れなど)、過小報告、未診断のエビデンスはなかった。実際、2009年にはMycobacterium tuberculosiscomplexの検査陽性率と結核疑い例における結核確定例の割合が低下しており、これは2009年の結核患者数の真の減少を示唆した。2009年の発症率の低下は小児(15歳未満)が最大であり(ジョージア州38%、ペンシルベニア州79%)、アメリカ国外出生者の発症率も有意な低下がみられた。以上の結果より、ジョージア州とペンシルベニア州の結核サーベランスシステムの機能は適切と考えられた。結核患者の診断や保健局への報告を含めた持続的な結核サーベイランスは、アメリカにおける結核発症率の低下の原因を解明し、結核の長期傾向を決定する上で有用である。
