HIV Testing and Treatment Among Tuberculosis Patients － Kenya, 2006-2009

結核患者におけるHIV検査と治療－ケニア、2006～2009年

世界中のHIV感染症患者において結核は死亡の主因であり、サハラ砂漠以南の地域における結核患者のHIV陽性率は80～90%と推定されている。2004 年にWHOがHIVプログラムと結核プログラムの協働促進を勧告して以降、多くの国々では結核/HIV協働活動が増加している。ケニアのDivision of Leprosy, Tuberculosis, and Lung Disease(DLTLD)は2005年に既存の結核サーベイランスシステムにHIV検査と治療に関する質問を加えたことから、この拡大結核サーベイランスシステムで2006～2009年に収集されたHIV関連データを分析した。新たに登録された結核患者の総数は、2006年の115,234例から 2009年には110,015例に低下した(低下率5%)。結核患者におけるHIV検査の実施率は2006年の60%から2009年には88%に増加し、結核患者におけるHIV感染症の有病率は52%から44%に低下した。HIV感染結核患者における2009年の日和見感染予防のためのコトリモキサゾール投与率は92%と高かったが(2006年:87%)、結核治療中の抗レトロウイルス治療(ART)開始率は34%のみであった(2006年26%)。ケニアでは、特に2009年WHOガイドライン(全てのHIV感染結核患者に対してCD4数に関わらずできるだけ早くARTを開始すること)を考慮し、HIV 感染結核患者のART実施率を増加させる方法を検討する必要がある。
