Update: Recommendations of the Advisory Committee on Immunization Practices (ACIP) Regarding Use of CSL Seasonal Influenza Vaccine (Afluria) in the United States During 2010-11

最新情報:2010～11年のアメリカにおけるCSL季節性インフルエンザワクチン(Afluria)の使用に関するAdvisory Committee on Immunization Practices(ACIP)勧告

2010 年のインフルエンザ期、オーストラリアおよびニュージーランドではCSL Biotherapies社製造の2010南半球季節性インフルエンザ3価不活化ワクチン(TIV、商品名:Fluvax Junior、Fluvax)が接種されたが、6ヶ月齢～4歳の小児において発熱や熱性けいれんの発症が増加し、市販後調査では、5～8歳の小児においても前年に接種されたワクチンに比べ、Fluvaxによる発熱の報告数が増加していた。FDAは2010-11年期における季節性インフルエンザワクチンとして同社のAfluriaを承認しているが、製造方法がFluvaxと同じであること、含まれるワクチン株はインフルエンザA(H3N2)ウイルス株とは異なるが、抗原性が同等であることから、とくに5歳未満の小児において発熱や熱性けいれんの発現率が高いことを添付文書に警告することとした。これを受けて、ACIPは以下の勧告を発表した。1)Afluriaは6ヶ月未満の小児には接種しない、2)6ヶ月齢～8歳の小児は他にその年齢を対象に承認され、発熱、熱性けいれんのリスクが低いワクチン製剤(TIV、弱毒性生ワクチン:LAIV)を接種する、3)インフルエンザ合併症のリスクのある5～8歳の小児で、他に適したワクチン製剤がない場合はAfluriaを接種してもよいが、親や保護者と利点とリスクについて十分に協議する、4)9歳以上の小児は Afluriaを接種してもよい。また、CDCは2010-11年期、Vaccine Adverse Event Reporting System(VAERS)、Vaccine Safety Datalink(VDS)を通じ、とくに9歳未満の小児における熱性けいれんの発症報告に十分注意し、定期的にレビューする予定である。
