Decrease in Reported Tuberculosis Cases － United States, 2009

アメリカの全国結核(TB)サーベイランスシステムの結果によると、2009年には50州およびワシントンDC からTB症例が11,540例報告され、発症率(10万人あたり)は2008年の4.2例から3.8例に低下した(低下率:11.4%)。2009年の発症率は1953年の全国TBサーベイランス開始以降最低であり、年間低下率は過去最高であったが(2000～2008年の平均年間低下率:3.8%)、依然として国内・国外出生者間および人種/民族間でTB発症率に格差がみられた。国内・国外出生者のTB発症率はともにかなり低下したが(2008年と比べた場合の低下率:それぞれ15.8%、9.0%)、それでも国外出生者のTB発症率は国内出生者に比べて約11倍高かった。また、ラテンアメリカ系と黒人のTB発症率は非ラテンアメリカ系白人に比べて約8倍、アジア系は約26倍高かった。TB症例のHIV重感染率は10.2%であった。疾患部位としては肺結核8,535例、肺外結核2,297例であった(培養陽性率:それぞれ83.6%、71.0%)。TB分離株の薬剤感受性については、2008年に感受性試験を行った培養陽性例の1.1%が多剤耐性であった(2007年:1.2%)。2009年のTB報告例の大幅な減少には人口統計学的な変化やTB対策の改善、未報告・未診断TB症例の増加などが関与している可能性があり、CDCは現在この急激な減少の原因を調査している。未診断・未報告TB症例はTB 拡大をもたらす可能性があるため、全てのTB疑い例は速やかに保健局へ報告する必要がある。
