医療補償範囲に関する法律の短期的効果－マサチューセッツ州、2008年

2006 年4月12日、マサチューセッツ州は全州民の医療保険へのアクセスを全て保障する法律を制定し、2007年7月1日より民間の医療保険および Commonwealth Careを通じ、州が補助する新しい医療保険制度が開始された。今回、Massachusetts Department of Public Health(MDPH)は、Behavioral Risk Factor Surveillance System(BRFSS)を通じ、この法律の短期的効果の分析を行い、その結果がまとめられた。調査は法制定前の2005年1月1日～2006年6月 30日と、法制定後の2007年7月1日～2008年12月31日の2期間に行われ、性別、年齢、人種、教育レベル、世帯収入、慢性疾患および加入する医療保険別の医療保険加入率が比較された。全体での医療保険加入率は91.3%から96.3%へ5.5%増加し、特にラテン系(14.2%)、高卒以下 (12.0%)、低所得(年収25,000ドル以下)世帯(11.9%)での増加が顕著であったが、それぞれの医療保険加入率はラテン系:89.0%、高卒以下:88.6%、低所得世帯:89.0%といずれも低いものであった。Commonwealth Careプログラムにより2008年までに新たに医療保険に加入した州民は約8%であり、公的保険加入率は法制定前:14.8%から制定後:19.2%と 29.7%増加し、民間医療保険は80.8%から78.2%へと3.2%減少、他の医療保険(個別保険、学生保険など)は4.4%から2.6%に減少した。かかりつけ医や医療提供者を持つ人は全体で86.1%から87.7%に増加、特にスペイン人(30.3%)、ラテン系(17.0%)での増加が顕著であった。また、健康診断を受けた人は71.9%から74.1%に増加、スペイン人(19.9%)、ラテン系(14.1%)にて顕著であった。以上から新しい法律の制定により、特にラテン系住民や低所得層での医療保険加入率が増加し、医療アクセスの増加につながっていることが示唆された。
