Global Measles Mortality, 2000-2008

2008 年、国連は麻疹による小児の死亡数を2000年の733,000例から2010年には73,300例以下と90%減少の目標を達成するコミットメントを再確認した。WHOとUNICEFは麻疹が問題となっている47ヶ国において、1)麻疹ワクチン(MCV)の2回接種率を90%以上とする、2)研究室レベルでのサーベイランスの強化、3)発症例に対する適切な医療措置の提供などの戦略を展開しており、ここでは2008年における現状を報告する。予防接種に関しては、1回目の接種(MCV1)率は83%(2007年:82%)、アフリカおよび東南アジア地域では依然として80%未満である(それぞれ73%、 75%)。2回目の接種はインドを除く46ヶ国にて追加接種活(SIAs)を介して約68,600万名の小児(9ヶ月～20歳)に接種された。また、 180ヶ国(94%)がWHOまたはUNICEFに麻疹サーベイランスデータを報告(2000年:169ヶ国)、症例数は2000年:852,937例から2008年:278,358例へ67%減少、地域別ではアメリカにて99%、アフリカにて93%減少したが、東南アジアでは3.6%の減少に留まっている。2008年の推定死亡数は164,000例、うち47ヶ国にて160,000例が死亡、77%(126,000例)は東南アジアにて発生している。今後の推定死亡数に関しては、最悪の場合(2009～2013年におけるMCV1接種率が2008年レベルを維持し、follow-up SIAsが行われない場合)、死亡数はリバウンドし2010～2013年において約1,700万名が死亡、現状維持の場合(インドを除く46ヶ国にて catch-up SIAが実施され、MCV1接種率が90%以上を超える国では2008年レベルを維持、90%以下の国では毎年1%増加する場合)、年 151,000～163,000名と推定される。2010年の目標を達成するには、インドにおける麻疹死亡率抑制策の適切な実施と、他の46ヶ国に対する経済的な支援と抑制策の継続的実施が不可欠であると考えられる。
