おたふく風邪のアウトブレイク－ニューヨーク、ニュージャージー、ケベック、2009年

おたふく風邪は発熱、唾液腺の炎症を特徴とする感染症であり、睾丸炎、聴覚障害、髄膜脳炎などの合併症を引き起こす。2009年8月、ニューヨーク州サリバン郡において約400名の男児が参加して開催されたある教団のサマーキャンプで、おたふく風邪のアウトブレイクが発生したとの報告があった。このアウトブレイクは6,584例が報告された2006年のアウトブレイク以降、最も規模が大きいものであり、10月30日の時点において確診例または疑われる症例はニューヨークおよびニュージャージー州にて計179例、カナダのケベックにて15例報告されている。index症例は11歳の男児、約4,000例のアウトブレイクが発生していたイギリスより6月17日に帰国しキャンプに参加、6月28日に発症し、その後キャンプの参加者およびスタッフ25名が発症[全例男、12(9～30)歳]した。ニューヨーク州ブルックリンではキャンプの参加者が帰宅した後に伝染が広がり、10月30日までに79例 [14(8～84)歳、男81%]に発症、他にニュージャージー州オーシャン郡にて40例、ニューヨーク州ロックランド郡にて27例、オレンジ郡にて8 例、カナダケベックにて15例の発症が報告されている。アメリカ国内での発症例(n=179)のうち、85例(47%)が研究室レベルにて確診され (IgM抗体およびRNAの検出)、94例(53%)は臨床症例基準を満たしている。性別が明らかな178例(99%)では149例(84%)が男性、年齢は14(8ヶ月～84)歳であり、ワクチン接種状況が明らかな141例(79%)では、2回接種:102例(72%)、1回接種:20例(14%)、接種なし:19(13%)であり、合併症は16例(9%)にて精巣炎(n=15)、一過性難聴(n=1)が認められ、死亡例は報告されていない。
