Effectiveness of 2008-09 Trivalent Influenza Vaccine Against 2009 Pandemic Influenza A (H1N1) － United States, May-June 2009

パンデミックインフルエンザA(H1N1)ウイルスは2009年4月の最初の報告後、世界中で伝播が拡大している。パンデミックウイルスは抗原性が季節性インフルエンザA(H1N1)と異なり、最近の血清学的研究ではパンデミックウイルスによる感染症に対して季節性インフルエンザワクチンは交差防御効果を示さないことが示唆されている。検査確定パンデミックインフルエンザA(H1N1)感染症に対する2008-09シーズンの3価季節性インフルエンザワクチンの効果を評価するため、CDCは症例―コホート分析を実施した。分析には、2009年5月～6月に8州でパンデミックインフルエンザA(H1N1)感染症と確定された18歳以上の患者356例のサーベイランスデータを利用した。これらの州における2008-09インフルエンザシーズン(2008年9 月～2009年2月)のインフルエンザワクチン接種率はBehavioral Risk Factor Surveillance Survey(BRFSS)の予備的データ用いて推定した。年齢群とインフルエンザによる合併症のリスク上昇が知られている慢性疾患の存在を調整後、パンデミックウイルス疾患に対する全体的なワクチン効果は－10%であった。この分析結果も含め、現在のエビデンスは季節性インフルエンザがパンデミックインフルエンザA(H1N1)の感染リスクに影響を及ぼさないことを示唆する。CDCと米国予防接種諮問委員会(ACIP)は2009-10シーズンにおける季節性およびパンデミックインフルエンザを予防するため、季節性インフルエンザワクチンとパンデミックインフルエンザワクチンの接種を推奨している。
