National, State, and Local Area Vaccination Coverage Among Children Aged 19-35 Months － United States, 2008

19～35ヶ月齢における国、州および地域のワクチン接種率－アメリカ、2008年

National Immunization Survey(NIS)による全国、州および選択地域における19～35ヶ月齢の小児におけるワクチン接種率に関する報告である。Health People 2010では4:3:1:3:3:1[ジフテリア/破傷風/百日咳(DTaP):4回以上、ポリオ:3回以上、麻疹/おたふく風邪/風疹(MMR):1回以上、Hib:3回以上、HepB:3回以上、水痘:1回以上]シリーズの個々の目標接種率が90%、シリーズ全体の目標接種率が80%とされている。今回、2008 NISデータベースより4:3:1:3:3:1シリーズおよび3、4回以上の7価肺炎球菌結合型ワクチン(PCV7)、2回以上のA型肝炎ワクチン (HepA)、生後3日以内のB型肝炎ワクチン(HepB birth dose)の2005年1月～2007年6月の接種率がまとめられた。4:3:1:3:3:1シリーズ全体の接種率は全国にて76.1%と2007 年:77.4%から減少、個々の接種率はDTaP:4回以上(84.6%)を除き全て90%以上であり、Hib:3回以上は92.6%から90.9%に減少していた。PCV7接種率は増加を維持(3回以上:2007年90.0%から2008年92.8%、4回以上:75.3%から80.1%)、 HepA:2回以上は40.4%、HepB birth doseは55.3%(2007年:53.2%)であった。また、全くワクチンの接種を受けていない小児は0.6%と2007年と同値であった。 4:3:1:3:3:1シリーズの州別接種率は59.2%(モンタナ州)～82.3%(マサチューセッツ州)、選択地域別では68.5%(カリフォルニア州北部の郡)～80.9%(カリフォルニア州サンタクララ郡)、HepB birth doseは19.1%(バーモント州)～81.4%(アリゾナ州)であった。また、人種/民族別接種率は黒人小児に比べ白人にてDTaP:4回以上、 PCV7:4回以上の接種率が高く、4:3:1:3:3:1シリーズの接種率は全ての人種/民族にて同等であったが、貧困層での接種率は有意ではないがいずれも低値であった。今回の調査ではワクチン接種率はHealth People 2010の目標値には達しておらず、今後も目標達成に向けて、特に貧困層での接種率を向上させていかなくてはならない。
