インドは野生ポリオウイルス(WPV)の伝播が続いている4ヶ国(アフガニスタン、ナイジェリア、パキスタン)の中で最も人口が多い。世界における最後の WPV2型感染症例は1999年10月にインドで報告された。一方、WPV1型、WPV3型はインド北部のUttar Pradesh州とBihar州で伝播が持続している。インドにおける土着のWPV伝播は、この2州以外の全州で2002年に遮断された。この報告は、 2007年1月～2009年5月29日のインドにおける予防接種活動、急性弛緩性麻痺(AFP)サーベイランス、WPV疫学について述べる。インドにおける3価経口ポリオワクチン(tOPV)の定期予防接種スケジュールは出生時、生後6、10、14週、16～24ヶ月である。2007～2008年の 12～23ヶ月の小児における生後12ヶ月時でのtOPVの3回以上接種率は66%であった。2005年、インド政府は追加予防接種活動(SIA)に tOPVよりもWPV1に対する効果が高い1価経口ポリオワクチン1型(mOPV1)の使用を導入した。2006年の多州にわたるWPV1のアウトブレイク発生後、mOPV1の優先的使用が促進され、WPV1症例は2007年の83例から2009年1～5月には18例に減少した。WPV3に関しては、 2007年のUttar Pradeshでの患者数再度増加によりBiharでアウトブレイクが発生した。しかし2007年に1価3型OPV(mOPV3)を使用したSIAが拡大実施され、WPV3症例数は2007年の794例から2009年1月～5月には41例に減少した。インドにおける全てのWPV伝播の遮断成功には、ハイリスク地域(Uttar Pradesh西部、Bihar)での同時伝播遮断が重要である。
