HIV Testing Among High School Students － United States, 2007

2006 年にはアメリカ住民約110万人がHIV感染症に罹患しており、そのうち232,700人は診断されておらずHIV感染を認識していなかった。HIV感染者のうち13～24歳の青少年・若年成人は全体の4.4%にすぎなかったが、未診断率は9.9%と高かった。HIV感染症の早期診断は治療介入を促進し、感染者のハイリスク行動とその後のHIV伝播の可能性を低下させることができる。青少年のHIV検査実施率を調査するため、Youth Risk Behavior Survey(YRBS)の2007年データ(最新データ)を分析した。50州およびワシントンDCの高校性(9～12年生:12,120人)の 12.9%はHIV検査を受けた経験があった。HIV検査の実施率は女生徒(14.8%)が男生徒(11.1%)よりも有意に高く、また人種/民族別では非ラテンアメリカ系黒人(22.4%)がラテンアメリカ系(12.7%)および非ラテンアメリカ系白人(10.7%)よりも有意に高かった。学年別では学年が上がるごとに実施率が増加し、12年生(18.9%)と9年生(9.1%)との間には有意差がみられた。また学校でAIDS/HIV教育を受けた群 (13.2%)は受けなかった群(9.7%)に比べて、性交歴のある生徒(22.3%)は性交歴のない生徒(4.0%)に比べて検査実施率が有意に高かった。性交歴のある生徒の検査実施率は、初回性交時年齢の上昇とともに低下した(13未満、13、14、15、16、17歳以上:それぞれ30.7、 29.5、23.3、20.7、15.9、12.6%)。青少年における未診断HIV感染症例数を減少しHIV予防を促進するため、CDCは健康管理者が定期診療の一部として13歳以上の全ての患者に対しHIVスクリーニングを実施するよう勧告している。また高校はHIV教育にHIV検査に関する情報を含めることにより、HIVスクリーニング対策を支援することが可能である。
