False-Positive Results with a Commercially Available West Nile Virus Immunoglobulin M Assay － United States, 2008

市販されている西ナイルウイルス免疫グロブリンMアッセイ偽陽性結果－アメリカ、2008年

2008 年9月、CDC、FDAおよび州保健省は市販されている西ナイルウイルス(WNV)免疫グロブリンM(IgM)捕獲型ELISA検査による偽陽性結果の増加に関する全国的調査を開始した。これは2008年夏、3つの州の保健省からCDCへWNV感染の臨床的および疫学的エビデンスのない患者がWNV IgM抗体検査で陽性を示したとの連絡があり、検査結果が全てInverness Medical社製のPanBio WNV IgM ELISAキットにより同じ大手の民間研究所にて得られていたことを受けての調査である。9月下旬、このキットのうち2つのロットの偽陽性率が表示 (2%、95%CI=0-9%)よりも高いことが確認され(20%、56%)、10月8日、製造者によりこの2ロットが自主的に回収された。56%と高い偽陽性率を示したロットは国内の4研究所に納品され、7～9月に検査に使用されていた。42州およびDCにて518例、568検体が陽性を示した。うち臨床情報の得られた488例のうち、83例(17%)がWNV神経浸潤性疾患症状、242例(50%)が非神経浸潤性疾患症状を示したが、163例 (33%)はWNV疾患の症状は認めなかった。10～12月、この568検体のうち166検体(29%)がCDCにてWNV IgM微粒子免疫蛍光アッセイ(MIA)およびIgM捕獲型ELISA法により再検査され、うち45検体(27%)が真陽性、119検体(72%)が偽陽性、2検体が不確定であった。また、これら518例のうち249例(28%)はArboNETに報告されていた。
