Potential Effects of Electronic Laboratory Reporting on Improving Timeliness of Infectious Disease Notification － Florida, 2002-2006

感染性疾患届け出の迅速性改善における電子検査報告の効果－フロリダ、2002～2006年

電子検査報告(ELR)は届け出疾患の報告とその後の疾患制御活動をより迅速化する可能性があるが、その効果や州および地域のサーベイランスチームの仕事量にもたらす影響は不明である。ELRの効果を検討するため、Florida Department of Health(FDOH)は潜伏期間の異なる4種類の届け出疾患(サルモネラ症[潜伏期間:1日]、細菌性赤痢[3日]、髄膜炎菌疾患[4日]、A型肝炎 [30日])の報告速度を評価した。ELRの効果は、ELRにより診断用検査の終了から郡保健衛生局(CHD)への結果報告までの期間が1日に短縮すると前提して算出した。2002～2006年に報告されたサルモネラ症23,263例、細菌性赤痢8,014例、髄膜炎菌疾患450例、A型肝炎2,104例を対象とし、発症日から検査結果報告日まで(A期間)、検査結果報告日からCHD届け出日まで(B期間)、発症日からCHD届け出日までの期間(C期間) を調査した。各疾患のA、B、C期間はサルモネラ症がそれぞれ6、5、12日、細菌性赤痢が5、4、10日、髄膜炎菌疾患が3、1、4日、A型肝炎が9、 3、13日であった。ELRによりB期間が全て1日となるため、C期間はサルモネラ症で12日から7日へ、細菌性赤痢で10日から6日へ短縮した。しかし B期間がもともと少ない髄膜炎菌疾患ではC期間に変化はなく(4日)、A型肝炎も短縮は少なかった(13日から10日へ)。以上、フロリダ州において届け出疾患報告の迅速化におけるELRの有用性は疾患によって異なることが示唆された。
