Smoking-Attributable Mortality, Years of Potential Life Lost, and Productivity Losses - United States, 2000-2004
喫煙による死亡率、損失生存可能年数、生産性損失-米国、2000~2004年
喫煙および受動喫煙は慢性疾患による早期死亡や社会経済的損失、健康管理システムへの多大な負担をもたらす。喫煙は全ての癌死の30%以上、慢性閉塞性肺疾患(COPD)による死亡のほぼ80%、早期心血管系疾患および心血管死に関与する。喫煙による経済的・公衆衛生的負担を評価するため、2005年に CDCは1997~2001年の喫煙関連死亡率(SAM)、損失生存可能年数(YPLL)、生産性損失に関する分析結果を報告した。その分析には、 National Health Interview SurveyのデータとNational Center for Health Statisticsの死亡証明書データに基づきSAM、YPLL、生産性損失を推定するCDCのSmoking-Attributable Mortality, Morbidity, and Economic Costs(SAMMEC)システムのデータが利用された。この報告は、SAM、YPLL、生産性損失に関する最新情報として2000~2004年の分析結果について述べる。この期間中、喫煙と副煙流曝露により少なくとも1年間に約443,000人(男性:269,655人、女性173,940人)が早期に死亡した。喫煙関連死の主因は肺癌(128,922人)、虚血性心疾患(126,005人)、COPD(92,915人)であった。35歳以上の喫煙関連死亡者(受動喫煙、家庭内曝露による死亡者を除く)のうち160,848人(41.0%)は癌、128,497人(32.7%)は心血管系疾患、 103,338人(26.3%)は呼吸器疾患であった。妊娠中の喫煙は年間で推定776人の乳児死亡をもたらした。また喫煙は年間約510万YPLL(男性:310万、女性:200万)の原因となり、年間約968億ドル(男性:642億ドル、女性:326億ドル)の生産性損失をもたらした。喫煙はいまだに健康および経済的な負担となっている。州は、包括的喫煙抑制プログラムへの出資をCDCの推奨レベルまで増加することによって喫煙率低下と喫煙による負担の軽減をさらに促進することができる。
