Federal Air Travel Restrictions for Public Health Purposes － United States, June 2007-May 2008

公衆衛生目的での連邦による航空旅行制限－アメリカ、2007年6月～2008年5月

伝染性疾患患者が民間航空機で旅行した場合、他の旅行者への感染リスクが生じる可能性がある。2007年6月、連邦機関はpublic health Do Not Board(DNB) listを作成し、特定の基準を満たし、社会に重大な脅威をもたらす伝染性疾患患者が米国から出発あるいは米国に到着する民間航空機に搭乗することを制限するよう、国内および国外の保健衛生局が要求できるようにした。public health DNB listは、CDCと米国Department of Homeland Security(DHS)によって管理されている。public health DNB listへの記載状況を調査するため、CDCは2007年6月～2008年5月のデータを分析した。この期間中にCDCが受け取ったpublic health DNB listへの記載要求は42例あり、このうち33例(79%)が基準を満たしリストに記載された。リストに記載されなかった9例中4例は地域保健衛生局による追跡対象者(隔離必要例など)であり、3例は民間航空機に搭乗しないことに同意し、2例は非伝染性と確認された。リストに記載された33例中28例 (85%)は米国の州・準州や地域の保健衛生局からの要求であり、3例はカナダ、1例はメキシコ、１例はU.S. Department of Stateからの要求であった。33例全例が肺結核(TB)の疑いあるいは確定例であり、年齢中央値は41歳(20～77歳)で、20例(61%)が男性であった。このうち18例(55%)はその後非伝染性あるいはTBでないことが確認されたため、リストから除外された。リスト記載から除外までの期間中央値は26日(2～193日)であった。public health DNB listにより民間航空機上での伝染性疾患患者から他の旅行者への感染を防ぐことが可能である。米国および他の国々の州および地域の保健衛生局はこの新たな公衆衛生ツールの有用性を認識するべきである。
