透析施設に対する感染症コントロールの必要性と非経口薬剤バイアルに関するガイダンスについて

2008年4月、Centers of Medicare and Medicaid Services(CMS)は連邦官報に末期腎疾患(ESRD)施設に対する保険保障に関する条件についての最終的な規則を発表した。2001年のCDC による「慢性血液透析患者における感染予防に関する勧告」がCMS補償範囲条件の新しい基準に組み入れられ、2008年10月14日よりESRD施設は外来透析サービスのMedicare報酬を受けるにはCDCの勧告に従うこととなった。近年、非経口薬剤バイアルの取り扱いの誤りからC型肝炎ウイルス(HCV)感染や他の細菌感染症のアウトブレイクが発生しており、2002年、CDCはCMSに対し使い捨てバイアルの使用を推奨したが、この時点ではCDCの勧告は採用されなかった。今回、全ての透析施設がCDCによる透析施設における標準予防策および感染コントロールに関する勧告に従うこととなり、とくにエリスロポエチンなどの単回使用の静注薬のバイアルは、1回以上穿刺してはならないとし、単回使用のバイアルに一度針を刺したら、そのバイアルの無菌性は保証されないとしている。また、透析施設は感染症や他の有害事象の発生を適切な地域または州の公衆衛生局に報告しなければならない。報告が遅れた場合は疾病アウトブレイクの認識やコントロールが遅れることとなる。
