偽陽性経口液迅速HIV検査－ニューヨーク市、2005-2008年

New York City Department of Health and Mental Hygiene(NYC DOHMH)は性行為感染症(STD)の無予約制クリニックを10ヶ所運営し、匿名でのHIV検査などの無料サービスを提供している。2003年には通常検査により33,375検体が検査され、552検体(1.6%)が陽性であった。2004年1月、このSTDクリニックでは、その場で行う迅速HIV検査(刺指により採取した全血検体を用いたOraQuick検査)を採用し、検査数は38,092件へ増加(偽陽性例は月平均1件以下)、2005年3月には経口液を用いたOraQuick Advance Rapid HIV-1/2抗体検査が導入され、検査数は47,024件へ増加した。抗体検査導入後7ヶ月における偽陽性検体は21,722検体中35件(0.16%)であったが、その後、偽陽性率が増加、2005年10月は11/4,024件(0.27%)、11月には36/3,735件(0.97%)となり、特異度が99.03%と製造者が下限とする数値を下回った。このため、NYC DOHMHは12月、経口液迅速抗体検査と刺指全血迅速検査を併用するよう指示し、2006年2月には経口液検査の特異度が99.65%に回復した。しかし、2007年後半になって再び偽陽性率の増加が認められ、2007年11月は23/4,503件(0.51%)、2008年2月には54/4,858件(1.11%、ピーク値)となり、2007年11月～2008年4月における検査特異度は98.88%～99.49%であった。このため、刺指全血検査の併用が再び指導され、5月には5/4,749件(0.11%)、特異度99.89%に回復し、ニューヨーク市では現在も経口液迅速抗体検査陽性例に対して刺指全血検査の併用を行っている。2002年以降、6種類のHIV迅速検査がFDAに承認されているが、CDCはこれらの検査による偽陽性検体について、増加の原因を調査し、偽陽性率を最小限とするためのプログラムの提供を進めている。
