State Medicaid Coverage for Tobacco-Dependence Treatments － United States, 2006

タバコ依存症治療に対する州のメディケイド補償範囲－アメリカ、2006年

成人メディケイド登録者の約1/3は喫煙している。Public Health Service、Task Force on Community Preventive Services、Institute of Medicineはタバコ依存症治療に健康保険を適用するよう勧告しており、Healthy People 2010はエビデンスに基づいた全てのタバコ依存症治療が全51州のメディケイドプログラムの補償範囲に含まれることを目標としている。メディケイドで補償されるタバコ依存症治療のタイプは、カリフォルニア州バークレイ校のCenter for Health and Public Policy Studiesで実施されている調査により、定期的に報告されている。この報告は、2006年の調査結果の概要を紹介したものである。Clinical Practice Guidelineで推奨されている全てのタバコ依存症治療(薬物療法、カウンセリング)を補償範囲としていたのは1州(オレゴン)のみであった。少なくとも1種類のタバコ依存症治療を補償していたのは43州で、そのうち39州(ワシントンDCを含む)は全てのメディケイド登録者、4州は妊婦のみを対象としていた。全登録者を補償範囲としている39州全てが、一部の薬物療法(塩酸ブプロピオン:37州、ニコチンパッチ:36州、ニコチンガム:34州、バレニクリン[2006年にFDAが承認]:32州、ニコチン鼻スプレー:30州、ニコチン吸入剤:30州、ニコチントローチ:28州)を補償していた。また、27州(全登録者対象:17州、妊婦のみ:10州)は禁煙カウンセリングサービスの一部を補償し、そのうち17州(全登録者:10州、妊婦のみ:7州)はグループカウンセリング、25州(全登録者:14州、妊婦のみ:11州)は個人カウンセリング、3州(全登録者)は電話カウンセリングを補償していた。2005年から2006年にかけて2州(アラスカ、マサチューセッツ)がタバコ依存症治療の補償を開始し、1州(デラウェア)はニコチントローチ、1州(オクラホマ)は個人カウンセリングを補償範囲に追加した。多くのメディケイドプログラムはタバコ依存症治療の補償に様々な制約があり、タバコ依存性治療を補償している43州中31州は共同支払いを必要とし、22州は薬物療法の期間に制限があった。喫煙者減少のためには、推奨されているタバコ依存症治療全ての保険補償、治療利用の制約の除去、治療の奨励、メディケイド登録者およびメディケイド関係者に対する保険補償に関する教育が重要である。
