Increased Antiviral Medication Sales Before the 2005－06 Influenza Season － New York City

2005～06インフルエンザシーズン前の抗ウイルス薬販売数の増加－ニューヨーク市

オセルタミビル、ザナミビル、リマンタジン、およびアマンタジンはインフルエンザA/Bの治療・予防用に承認されている抗ウイルス薬であるが、循環しているインフルエンザA株の耐性レベルが高いため、2005年1月にCDCは2005－06インフルエンザシ－ズン中にアマンタジンとリマンタジンをインフルエンザAの予防や治療に使用しないよう、暫定的な勧告を発表した。症候サ－ベイランスの一部として、New York State Department of Health（NYSDOH）はメディケイドシステムによって支払われた抗インフルエンザウイルス薬の販売数を、New York City Department of Health and Mental Hygiene（NYCDOHMH）は小売薬局チェ－ンによる抗インフルエンザウイルス薬の販売数をモニタリングしている。2001－02、2002－03、2003－04、および2004－05の各インフルエンザシ－ズン中、抗ウイルス薬販売数のピ－クは検体のインフルエンザ陽性率のピ－クと一致した。しかし2005－06シ－ズンは、WHO研究所による最初のインフルエンザウイルス循環エビデンスの報告（2005年12月11日－17日:50週）の7週間前の2005年10月23日－29日に抗インフルエンザウイルス薬販売数が急増した。販売数が急増した時期は、鳥インフルエンザA(H5N1)とインフルエンザのパンデミックの可能性に関するメディア報道が開始された時期と一致していた。他のインフルエンザ流行マ－カ－（介護施設などでのアウトブレイク、医師の報告、救急部受診、肺炎およびインフルエンザ死亡率）は、2005年10月中にNYCにおける2005－06インフルエンザシ－ズンの開始の兆候を示さなかった。処方薬と一般用医薬品の販売追跡は、直接的徴候が存在しない段階で急増を検出できる可能性がある。このような徴候デ－タは、インフルエンザの流行を監視している研究所、健康管理施設、健康管理者によって提供されたデ－タと組み合わせた場合、薬剤の供給制限や個人での薬剤の保管が妥当的ではないとする公衆衛生勧告の必要性の判断に有用と考える。
