新生児/産科病棟における結核菌伝染－ニューヨーク市、2003年

2003年12月、ニューヨーク市内の病院Aの新生児/産科病棟に勤務する看護師(女性、フィリピン出身)が肺結核と診断された。この看護師は11年前にツベルクリン反応(TST)にて硬結＝15mmのため不顕性結核感染症(LTBI)と診断されたが、BCG接種によるものと判断し、イソニアジドは投与されておらず、その後の定期的結核検査でも徴候は認められていなかった。2003年9月、喀痰を伴う咳、喘鳴、息切れのため胸部X線検査を受けたが、異常なしと診断され、抗喘息薬による対症療法を受けた。しかし症状が持続したため、約8週間後にCTおよび気管支鏡検査を受け、さらに経気管支生検の結果、M.tuberculosis(遺伝子型はNYCおよびアメリカのデータベースに一致せず)が検出された。この看護師の感染期間(9月1日－11月29日)において、接触したと思われる同僚:32名、新生児:613名、産科病棟入院例900名を対象に調査した結果、同僚での感染は認められず、また、連絡が取れた新生児、産科病棟入院［それぞれ227(37%)、216(24%)］では、新生児:5/227名(2%、1名はBCG接種済み)にてTST陽性(＝5mm)が認められ、帝王切開との関連が疑われた。入院例では19/216(9%)がTST陽性を示したが、看護師との接触との関連性は認められなかった。TST陽性例(およびHIV感染陽性例)にはイソニアジド9ヶ月間投与によるLTBI治療が勧告された。
